Please note that the Office of Vital Statistics only maintains vital records for births, deaths, and marriages that have occurred in the City of Passaic.

Please note that this office does not maintain records for births, deaths, and marriages that have occurred in other County of Passaic municipalities. Such requests must be directed to the municipality where the birth, death, or marriage took place.

The Office of Vital Statistics only accepts in-person and mail requests for copies of vital records.

For in-person requests, the Office of Vital Statistics is open Monday to Friday (excluding legal holidays), 8:30 AM to 4:00 PM. The Office of Vital Statistics is located in Passaic City Hall, First Floor, 330 Passaic Street, Passaic, NJ 07055.
